The referral form is utilized to refer clients to coaching or therapy at Modern Health. This form is located in the Resources page of the provider dashboard. It is two pages long and it will require you to completely fill out the first page before advancing to the second.
It is important that you only fill out and submit the referral form for clients who have consented or who have been notified. Prior to completing the form, you want to make sure you have all required information about the client that is being referred to ensure our care matching team has all of the necessary details to find the best match possible.
You will receive a notification following the submission of the form, as well as a notification via email after you submit this form indicating that our care team has received your request and is working to select the best match for your client.
