How do I report a technical bug?

Use the Issue Reporting Form to report product/technical bugs with Modern Health. Before submitting, check the Help Center for answers. If still unresolved, access the form via the Resources page of the provider dashboard or the direct link below. Provide detailed information about the issue, including screenshots when possible. After submission, the Modern Health team will review your report and follow up by email to offer further assistance.

 

The Issue Reporting Form should be utilized when you need to report a client issue and/or possible bugs with the Modern Health product. We encourage you to leverage the articles in this Help Center first to ensure the answer is not already available. 

After identifying an issue to report, you can navigate to the Resources page of the provider dashboard, or simply click here, to access the Issue Reporting Form. 

 

We ask that you include as much detail as possible surrounding the issue in the form and include screenshots when applicable. After submitting the form, the Modern Health team will review your submission and reach back out via email to further assist you.
