How do I report a client issue or technical bug?

Please use these forms to report a technical issue and/or product feedback with the Modern Health product. When filling out the form, please include as much detail as possible, and where applicable, include screenshots. The Modern Health team will review your submission and reach back out via email to help resolve the issue! 

 

To locate the forms, please navigate to your Provider Dashboard, select the resources tab, and select the Issue Reporting card.

If you are on the Invoicing only dashboard or can not access this card in the full provider dashboard please report the issue you are facing in detail to our Provider Success Team at provider@joinmodernhealth.com.
