Please follow these steps to reset your password and access your provider dashboard:
- Using Chrome or Firefox as your browser, navigate to chat.joinmodernhealth.com
- Select “Forgot your Password?"
- Enter the email you have on file with Modern Health. You will receive an email in your inbox with a link to reset your password to allow you to log into the dashboard.
Kindly note that password reset emails can take up to 24 hours to arrive in your inbox. If you are accessing the above URL using Chrome or Firefox and your attempt to reset your password still fails to deliver you an email, then please let our team know (provider@joinmodernhealth.com) and we will look into it further.
