The Consent for Treatment Form is integrated into the member’s pre-session process to help support consistent and compliant clinical documentation. Modern Heath’s Consent Form covers all care under Modern Health as a practice and is not specific to individual providers.
A provider can view a member’s consent status in the ‘Forms’ tab of the member’s profile within the Provider Dashboard to ensure it’s completed prior to the first appointment.
Consent for Treatment Forms are automatically sent to the member when the Initial Evaluation is scheduled, reducing manual steps for providers. If a member’s Intake Form remains unsigned 24 hours prior to the first session, providers will receive an email notification so that they can prompt a member to complete the Consent Form. Providers receive email notifications about outstanding forms 24 hours before the first session so they can prompt a member to complete it.
Best practices
- Verify the completion status of the consent form in the ‘Forms’ tab within the Provider Dashboard ahead of the first session.
- If the Consent Form is ‘Not Signed’, providers should message the member promptly and, if needed, reschedule treatment until the Consent Form is signed, unless exceptions to applicable consent requirements apply.
Step-by-Step Guide: Consent Forms Workflow
1. Consent Forms will be sent when:
- An appointment is scheduled with a member who does not have a signed Consent Form. Remember that the Consent Form covers all Modern Health providers and is not unique to each provider.
- Important to note:
- Modern Heath’s Consent Form covers all care under Modern Health as a practice and is not specific to individual providers
- At this time, for all existing matches, Modern Health will assume that providers have collected consent for treatment already.
- Members will be able to access the form in app and will also receive email reminders to sign the form before their initial appointment.
As a therapist, you’ll receive email notifications in two cases:
- When a member who is sent a Consent Form has completed the Consent Form prior to their initial session.
- If a member with an unsigned Consent Form 24 hours before a session still hasn’t completed the Consent Form.
2. Reviewing Completed Consent Forms
- Providers are able to view the status of the Consent Form within the ‘Forms’ tab of the member profile.
- Only Therapists assigned to the member can view completed forms.
- Coaches do not have access to member forms.
- Some members may not have a consent form in the ‘Forms’ tab, which is okay. This likely means that their consent form has been completed outside of Modern Health prior to the launch of Modern Health’s EMR.
Consent form statuses
- No consent form needed: If the ‘Forms’ tab has no consent form card, the member doesn’t need a new consent form. You can assume that the consent form you have collected from them before the launch of the Modern Health EMR is sufficient. Please continue care as normal as the member’s consent is not overdue.
- Unsigned: If the consent form card shows “Unsigned,” please obtain the member’s signature before treatment, unless it’s an emergency.
- Signed: If the consent form card shows “Signed,” proceed with treatment as normal.
