The Member Chart contains the demographic information for a member you’re actively matched with. The Member Chart is located in the member’s profile within your Provider Dashboard. You can navigate there by clicking into the member’s profile. The information the member completes within the Intake Form will auto-populate within the Member Chart. If any demographic information is missing, please gather it during the initial evaluation. A provider can update the Member Chart at any given time by clicking on the ‘Edit’ button. The information in the Member Chart also exists in the member application and there is bidirectional edit functionality; that is, when a field is updated in the Member Chart, it is visible in the member interface, and vice versa.
