Some of your Modern Health clients may be eligible to access additional therapy sessions through employer-sponsored health plan integration. To submit claims on your behalf, we require a few additional pieces of information which we will include on all invoice submissions for therapists. This will include asking for diagnosis or DSM code and CPT codes (diagnostic and therapeutic procedure codes).
Note: this step will only appear on the submission form for overage sessions (it will remain unchanged for all other sessions through Modern Health).
Please read on for the three steps required to complete the invoice submission:
1. Begin by completing the basic information about the session (Client name, session date and time, attendance, and modality).
Note: Modern Health will still reimburse late cancellation and no-show sessions, and in these cases, there will be no need to complete Steps 2 and 3.
2. Complete the billing details by selecting a CPT and Diagnosis code. Commonly used CPT codes will appear at the top of the drop-down menu.

3. Rate your session and share feedback if desired.
4. Once you have completed the form, select "Submit" and an invoice confirmation popup will appear.
