Coach Care Forms are Modern Health’s intake forms for members. They allow clients to reflect on their current state and share key information before coaching sessions. Members are prompted to complete the form after matching with a new coach and scheduling a session. The Care Form is also accessible via the “Complete Care Form” option in the “Your Next Step” section on the web or app. Submitted Care Forms cannot be revisited or shared with other providers.

As a coach, you will receive a notification via email when a new client match has completed their care form. We have updated your client profile view in the provider dashboard to contain space for your client’s care form. Within the client profile, you will be able to view the status of the form (Completed or Incomplete) and click into it to view the responses. Below you will find screenshots of how it will look within your provider dashboard:
Clients will be prompted to fill out the form in multiple ways between the initial match and the first scheduled session. Once the form has been completed, you will receive an email notifying you as such and you can proceed to review the responses in preparation for the first session.
With new clients taking the time and effort to reflect within the form, you will want to be sure you review the responses ahead of the first session. The goal is to build a stronger connection with new clients and increase session engagement. To do so, we have listed out a few best practices when preparing for first sessions:
- Be sure to set aside some time prior to the first session to ensure you can properly review the completed form.
- Note down your key observations or things to clarify.
- Be prepared to discuss the completed form in the session.
- If the client did not fill out the form or skipped some sections, you can offer to go through the questions during your first session together.
- Let the client know that you can revisit the intake form together in subsequent sessions to ensure that the client is making progress towards their stated goals.
