This article provides step-by-step guidance to help you activate and verify your Hyperwallet account as a U.S. citizen or Tax-paying Resident so you can receive payments from Modern Health. Please complete all steps carefully, as accurate information is required to ensure successful activation, tax reporting, and smooth payment transfers.
Hyperwallet is Modern Health’s secure payment platform for providers. It complies with robust security and privacy standards, including PCI Level 1, SOC 1, SOC 2, and ISO 27001 certifications.

To begin receiving payments, you must:
- Activate and verify your account using a phone number on file.
- Set up your Hyperwallet account and login credentials.
- Complete Taxpayer Verification (Substitute Form W‑9 for U.S. tax reporting)
Step 1: Receive Your Hyperwallet Invitation
You will receive an email invite from Hyperwallet at one of the following email addresses:
The invitation will contain a link to begin the activation of your Hyperwallet account.
Please note that this is a temporary link. After you activate your account, or after 3 days, the link will no longer be accessible.
After activation, you can log in to your Hyperwallet account at this Login Portal.
Step 2: Accept the Invitation
- Open the email from Hyperwallet.
- Click “Activate” in the body of the email to start the setup process.
Step 3: Verify Your Account via Phone Number
For security purposes, Hyperwallet will prompt you to verify your account using a phone number associated with your Modern Health profile:
- Select “Verify my account using my Phone Number.”
- Enter your phone number and click “Continue.”
If you receive an error stating “Unfortunately, we are unable to complete your activation. Please contact our Support Center to assist you with the activation process”, it likely means Modern Health does not have a phone number on file for you.

If verification fails:
- Try another phone number that may be on file with Modern Health.

In this section, you will complete your electronic tax documentation to ensure accurate IRS reporting.
Step 1: Accept eCommunications Disclosure
- Review and accept Hyperwallet’s eCommunications Disclosure policy.
- This policy allows Hyperwallet to communicate with you about your account and tax information electronically.
- After agreeing to the Terms and Conditions, click “Continue.”
Step 2: Complete Substitute Form W‑9
- You will be prompted to fill out the electronic Substitute Form W‑9 with:
- Your first and last name
- Your SSN
- The address associated with your IRS records
- If you need to update the pre-populated first or last name, or your address:
- Go to “Settings” → “Profile” in the main navigation bar.
- Update your information and click “Save.”
- To submit the form electronically:
- Accept the Terms and Conditions,
- Provide your typed signature, and
- Click “Confirm.”
Step 3: Complete Taxpayer Verification
After submitting your information, Hyperwallet will attempt to verify your taxpayer details:
- If you see “Your taxpayer information has been successfully verified”, click “OK.”
- If you see “Your taxpayer information could not be verified”:
- Navigate to “Settings” → “Profile” and confirm that your name, SSN, and address match your IRS records exactly.
- Once updated, go to “Home” in the main navigation bar.
- You will be prompted again to accept the eCommunications disclosure and re-complete the Substitute Form W‑9.

Once your taxpayer information is successfully verified, you will be able to transfer money out of Hyperwallet and ensure accurate income tax reporting.
Group practices and company entities will receive an invite to set up their Hyperwallet account under the company name that was provided to Modern Health.
If you would prefer for your Hyperwallet account to be set up under an individual name and tax information rather than a company name and tax information, do not go through the activation/verification process. Instead, please reach out to us at onboarding@joinmoderhealth.com (for our provider applicants) or provider@joinmodernhealth.com (for our active providers), and the appropriate Modern Health team will help you adjust your setup.
Step 1: Enter Business Profile Information
- Enter your legal business name (if this appears uneditable, you may be able to update it later), DBA/Trade Name, business type, EIN, business registration number, business registration company/region, and company address.
- Click “Continue.”
As with individual accounts, Hyperwallet provides robust security, privacy, and compliance controls, and more details are available via their Security & Compliance site.
Step 2: Enter Business Contact Information
Provide details for the person who will manage the practice’s Hyperwallet account:
- First and last name
- Date of birth
- Nationality
- Phone number
- Business address
Then click “Continue.”
Step 3: Create Login Credentials and Security Questions
- Choose a password and security questions for the practice’s account manager.
- You will be asked one of these questions each time you log in or contact Hyperwallet support, so please store your answers securely.
- Review and accept the Terms and Conditions, and click “Continue.”
Your Hyperwallet account for the group practice/company is now activated, and you can add payment transfer methods. As with individual accounts, you must still complete Taxpayer Verification before you can transfer funds out of your account.
Taxpayer Verification for Group Practices and Companies
Step 1: Accept eCommunications Disclosure
- Review and accept Hyperwallet’s eCommunications Disclosure policy, which enables electronic communications regarding your account and tax information.
- After agreeing to the Terms and Conditions, click “Continue.”
Step 2: Complete Substitute Form W‑9 (Business)
- Fill out the Substitute Form W‑9 with your:
- Taxpayer Identification Information
- Taxpayer Identification Number (e.g., EIN)
- Exemption codes, if applicable
- Taxpayer address information
- If you need to change information that appears uneditable:
- Go to “Settings” → “Profile” in the main navigation bar.
- Update your information, ensuring that your Legal Business Name and EIN exactly match your IRS records, and click “Save.”
- To complete the electronic submission:
- Accept the Terms and Conditions,
- Provide your typed signature, and
- Click “Confirm.”
Step 3: Complete Taxpayer Verification
After submission:
- If you receive the message “Your taxpayer information has been successfully verified”, click “OK.”
- If you receive “Your taxpayer information could not be verified”:
- Wait 24 hours, then navigate to “Home” in the main navigation bar and resubmit your updated information.

Once verified, your group practice or company account will be eligible to transfer funds out of Hyperwallet, and your tax information will be correctly reported.

I’m an individual provider. Should I set up my account under my personal or business information?
By default, individual providers set up Hyperwallet using their individual name and tax information. If you instead prefer to be set up and paid under a company name and tax information, do not complete the activation steps and email payments@joinmodernhealth.com so our team can support that change. -
I’m part of a group practice. How should we set up our Hyperwallet account?
Group practices receive an invitation to set up Hyperwallet under the company name. The person who will manage payments for the practice should complete the business profile, contact details, and taxpayer verification using the practice’s legal business information. -
