Modern Health Care Partners for USA-based members only

Modern Health aims to make mental health more accessible by connecting clients with high-quality providers. We have partnered with Charlie Health, which offers intensive outpatient programs for youth aged 11-50, and Equip, a treatment program for eating disorders serving ages 6 and older. Eligibility for these programs varies by location and other factors; Charlie Health serves members in 33 states, while Equip is available nationwide. Referrals can be made through the two separate platforms. 

What do Charlie Health and Equip do?
  • Charlie Health is a virtually based intensive outpatient (IOP) program for youth and young adults (ages 11-50).
  • Equip is an eating disorders treatment program for youth and adults (ages 6+).
How do I know which of my Modern Health clients are eligible for these benefits?
  • Charlie Health is available for Modern Health members ages 11-50 who reside in one of 33 US states: Alaska, Arizona, California, Colorado, Delaware, Florida, Georgia, Idaho, Illinois, Indiana, Iowa, Maryland, Michigan, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Ohio, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Washington, Wisconsin, Wyoming.
  • Equip is available for Modern Health members ages 6+ in all 50 US states.
How do members pay for these services?

Please note that individual eligibility will vary based on health plan (insurance), diagnostic criteria, and other factors determined by Charlie Health and Equip. You do not need to screen members for these factors before making a referral.

How do I know if my member has access to these services? [Invoicing only Dashboard]

If you are on the invoicing-only version of the Dashboard, you won't be able to share benefits resources with clients via our platform. However, if any of your Modern Health clients meet the eligibility criteria above you can refer them to these partners for care.

You can either share the referral links with your clients via email; or you can use the same referral link to issue the referral on their behalf.

How do I know if my member has access to these services? [Full Provider Dashboard]

You'll be able to browse the client's available benefits by: 

  1. Click into the member's name 
  2. Scroll down to the Member Information section  
  3. Click into Company Benefits
 
 
 
 

 

How do I refer an adult (ages 18+) client?

You can issue the referral by submitting the form available in the Charlie Health or Equip website, for clients that feel uncomfortable doing it themselves. Alternatively, you can share the link with information on the benefit via the Messages tab where the member will have access to the self-referral form that they can fill out directly.

It's important to note that Modern Health does not guarantee placement in a program. After a member is referred they will need to go through the partner's intake which will then determine if they meet the criteria to be admitted.

As such, Modern Health also does not have any visibility or oversight into how the referrals are processed or what stage of review they are in. If a member is referred to a clinical partner, they will need to directly contact the partner's support team for any information pertaining to their referral and/or treatment. If it feels appropriate, you are welcome to check in with the member after a few weeks to see how the referral went, but we do not expect you to track or manage the referral.

How do I refer a minor client if they don't have access to the Modern Health app?

You can issue the referral by submitting the form available in the Charlie Health or Equip website, for clients that feel uncomfortable doing it themselves. Alternatively, you can share the link with information on the benefit via email with the minor directly, or with their parent/guardian. That link gives them access to a self-referral form that they can fill out.

Modern Health does not guarantee placement in a program. Charlie Health and Equip are third party clinical partners of Modern Health. The information collected by a third party clinical partner will be subject to that partner’s Privacy Policy and HIPAA Notice of Privacy Practices. Any clinical services subsequently provided by a third party clinical partner will be governed by that partner’s Terms of Service.
