How do I update/share new licenses or ICF credentials?

In addition to your short-bio, your License updates (for therapists) and ICF Credential updates (for coaches) are not able to be updated directly on the Dashboard. You should continue to contact Provider Support for these updates via provider@joinmodernhealth.com.

 

Therapists

US and Canada therapists - New License

Please use this form to add a new license to your account. After you submit it, please reach out to Provider Support via email stating that you've submitted the form, and sharing:

  • the new state/province you are licensed in; 
  • your license type;
  • your license number

Our team will confirm that your submission was successfully received via the form, ensure that the new license being added is reflected in our license monitoring system, and that your profile in our database is updated to include your new licensed location so you can be eligible for referrals based in the new location.

Non-US or Canadian therapists - New License

Please reach out to Provider Support via email and share the following:

  • the new province or country you are licensed in;
  • your new license/registration/certificate number;
  • a copy of your license/registration/certificate (as applicable)

Our team will receive your new license information and will submit for vetting and approval by our Compliance team. If the new license is approved, we will update your profile in our database to include the new licensed location (country) so you can be eligible for referrals based in the new location.

Renewal of Existing License

If you want to share a renewal of an existing license, please reach out to Provider Support with a screenshot of your renewed license.

Our team will need to verify that your renewed license is accurately captured in our license monitoring system.

[US-only] PSYPACT

Please email Provider Support and share the following:

  • screenshot of your approval email;
  • screenshot containing your APIT Number;
  • a list of the subset of PSYPACT states in which you'd like to offer care, or confirm that you are open to offering care in all of the participating PSYPACT states.

 

Coaches

Please reach out to Provider Support via email with a screenshot of your updated credential (or new credential, when applicable).

Our team will ensure that your profile in our database is updated to capture those changes.
