Updated Provider Agreement FAQs - October 26th, 2021

Providers who joined Modern Health's network on or before October 26th, 2021 received updated provider agreements via DocuSign. Please read on for some FAQs regarding the new agreements. Please note, we distributed the agreements in a phased approach, and applicable providers received an email with the subject line "[Please Read] - Important updates to your provider agreement" containing more details on your individual timeline for completing the agreement via DocuSign. 

 

What are the main changes in the new provider agreement?

The following is a summary of select changes that we are highlighting for your convenience. Please note, the below summary does not include all changes made to the agreement; you will need to thoroughly read the entire document after receiving it via DocuSign:

  • Contracting Parties 
    • Due to a change in our operating model, this section updates the Agreement to reflect that providers will now be independent contractors, contracting with Modern Health Arizona P.L.L.C., a professional limited liability company, in place of Modern Life, Inc. and Modern Health.
  • Documentation; Access to Records (Article 2 Section I)
    • Contains a new section with expectations around documentation of services and access to records.
       
      • At no point is identifiable member information, including diagnosis or specific usage data, shared with any member’s employer.
         
  • Provider Access (Article 3 Section C)
    • Contains a new section clarifying Modern Health’s storage of client records as required by law.
  • Billing and Rights to Charges for Services Rendered (Article 14)
    • Contains a new section regarding cooperation with Modern Health in completing records, reports, or claims forms.
       
  • Provider Terms of Use
    • When you use the Modern Health provider portal, you will have to agree to Modern Health’s Terms of Use. These terms outline the expectations for using the Modern Health website, provider portal, app, and other services provided by Modern Health when you engage with our members.
What is the timeframe/deadline for signing the agreement?

We will notify you when you’ll be receiving the agreement and then you will have approximately 4 weeks from receipt of the agreement to return it back to us. We will give you reminders weekly if you haven’t signed the agreement yet as well.

How do I ensure successful completion of executing the agreement?

Once you have signed the agreement in DocuSign, you will receive an email notification from DocuSign informing you that you have successfully signed the e-document. Additionally, the email will include an attachment of the completed document. We suggest that you download and save the completed agreement for your reference.

What if I don’t want to sign the new provider agreement?

We hope that the new agreement is still acceptable for all of our providers, but in the event that it’s not, please reach out to us at provider@joinmodernhealth.com to see if we can accommodate the change(s) you’re looking to make.
