[Coach Only] How do I view my client's care form?

When a member matches with a new coach and schedules a session, they will be prompted to fill out the care form. They will have the opportunity to fill out the form in one sitting or save and return later. The form is entirely optional for the member and any of the questions can be skipped if desired.

 

Below you will find the questions contained in the form:

  1. What has motivated you to seek coaching? Fill in the blank

  2. Have you experienced coaching and/or therapy before? Multiple choice

  3. How would you describe your personal and professional life right now? Fill in the blank

  4. How can I support you in coaching? Rate the following (1-5)

    • Accountability

    • Brainstorming

    • Exploring and removing obstacles

    • Insight into your potential

    • Vision

    • Encouragement and inspiration 

  1. What are the top 2-3 specific outcomes that you want to achieve from coaching? Fill in the blank
  2. At the end of your sessions, how would you like to describe your personal and professional life? Fill in the blank 
  3. Anything else that’s on your mind? Fill in the blank 

As a coach, you will receive a notification via email when a new client match has completed their care form and the client's profile will update to reflect this.  Within the client profile, you will be able to view the status of the form (Completed or Incomplete) and click into it to view the responses. Below you will find screenshots of how it will look within your provider dashboard:

Clients will be prompted to fill out the form in multiple ways between the initial match and the first scheduled session. However, if a client does not complete the form, it will not prevent them from having their first session with you.

With new clients taking the time and effort to reflect within the form, you will want to be sure you review the responses ahead of the first session. The goal is to build a stronger connection with new clients and increase session engagement.
