Modern Health prioritizes continuity of care by allowing providers to request additional sessions for clients experiencing significant distress when two covered sessions remain. Providers must submit requests via the Additional Session Request Form, specifying the client’s needs and the number of extra sessions needed. It is solely the provider’s responsibility to facilitate these requests; clients should not be directed to HR or Benefits, ensuring their mental health information remains confidential within their organization.
Best Practices for Requesting Additional Sessions
Some employers that partner with Modern Health may cover additional sessions for their employees. As such, our recommendation is that you always submit an Additional Session request for your Modern Health clients prior to transitioning clients to private pay, or discussing other options. This will allow you to tap into any additional employer-sponsored coverage the member might have available to them. This ensures the member has continuation of care, while also preventing them from having unnecessary out-of-pocket costs.
When Should I Request Additional Sessions?
We recommend that you submit the request when the client is 2 sessions out from exhausting their available balance. This will ensure that your request is received, reviewed and responded to sufficiently in advance to allow you to use the member's remaining sessions to plan the continuation, termination or transition of treatment (depending on the result).
Where can I find the Additional Sessions Request Form?
Coaches and other Providers on Full Dashboard
Providers with access to the full dash will be able to access the Continuity of Care/Additional Session Request by logging on to their dashboard, navigating to the Resources tab, and selecting the Continuity of Care card.
Invoicing only Dashboard
Please submit a request for additional sessions through the Additional Session Request Form. Please save this form to your favorites or wherever you are most easily able to access this form to request additional sessions for your member. You are able to access a link to this form in our Coach Policies & Procedures or in our Therapist Policies and Procedures.
Our team will process your request once you submit it via the form. We will review each request based on the employer’s policy and will follow up with you to inform you of the outcome.
